On OS X 10.6 twain-sane doesn't install properly via macports so you have to download and go through several next-next-finish clicks (painful, I know)

[Edit: put steps 2 and 3 in the correct order]
Install libusb.pkg
Install sane-backends.pkg
Install twain-sane interface.pkg
Reboot (I did, maybe it isn't necessary)
Open Image Capture
open /Applications/Image\ Capture.app
Click SANE
Rock and Roll (and scan)
